UNC Chapel Hill Libraries Request Form
You may use this form to request that the Library send notices to you by email rather than by surface mail.
Library notices include:
- overdue notices
- recall notices
- pickup notices
- fine notices
- lost item notices
The Library must send all notices to you by surface mail or send all notices to you by email. If you
choose to receive notices by email you will not receive paper copies of Library notices.
I request the Library send all my notices by email and not by surface mail. I understand all notices
will be sent to the email address I have provided. I will notify the Library if I change my email address, or if I decide to revert to notification by surface mail.
