The call slip indicates the name, number, and portion of the collection you wish to use. When you fill out your call slips, use the finding aids to gather all the necessary information, and consult the public services staff for assistance. Call slips are available in the Reference Room.

When filling out call slips be sure to:
- provide the collection or call number;
- include the collection name if applicable;
- indicate the specific portion by circling the container type and listing the container number;
- include a series or sub series number if applicable;
- circle a format type if you are requesting an SFC recording or SOHP interview;
- complete a separate call slip for each collection you wish to use;
- complete a separate call slip for each format (such as listening copy and liner notes) you wish to use; and
- print your first and last name and the date of your request.
