Reserve Material Request Forms

The library provides multiple ways to access reserve material request forms. In addition to the downloadable files below, forms can also be obtained from the Reserve Desk in the House Undergraduate Library.

You may download the forms below, fill them out using Microsoft Word, print and mail to the Undergraduate Library at CB #3942 or fax to 962-2697.

You may also email the completed forms and appropriate accompanying PDFs to reserve_reading@unc.edu. If you need to bring physical items, such as books, to the library to complete your list, be aware that we will not begin processing your list until we have received all materials.

Request Forms for New Reserve Items

  • Photocopies : Use this form to request that the photocopies you submit are placed on electronic and/or paper reserve in the library. Examples of items for which photocopies are often submitted include: journal articles, book excerpts, and quizzes. Items must comply with copyright policies in order to be placed on electronic reserve.
  • Books: There are two ways to submit books for Reserve. Books or coursepacks that are owned by the library or by the instructor can be submitted using the Books Reserve Form. Books can also be submitted by using the UL Reserves Tool located in the library catalog. (See instructions). For copyright reasons, we cannot scan whole books for electronic reserves. Book excerpts can be submitted via the Photocopies form above. If you place a personal copy of the book on reserve, please be aware that it may be damaged or lost by patrons using the item.
  • Links : If electronic books or articles are located in the online library databases or on the open web, Reserves can add links to these items in your electronic reserves. Since no photocopies are submitted with this form, there will be no photocopy on reserve here in the library for check out or as a backup if the link no longer works.

Renewal or Changes Forms:

  • Renewals : If you currently have items on file, you may renew them using this form. If your course is canceled, please let us know as soon as you can.
  • List Changes : If you would like to change the loan period for an item, transfer the list to another instructor, or remove single items rather than an entire list, please use this form. If you would like to remove items, you may also email us with the items listed that you would like removed.

Contact Reserves Staff (reserve_reading@unc.edu) if you have questions about request forms. Only items requested on the Photocopies or Links forms are eligible for e-reserve, pending evaluation of fair use and permissions restrictions.
